UPDATE: HSE confirms deaths of two mental health patients

The review was confirmed in a statement tonight.

THE HEALTH SERVICE Executive is investigating two separate deaths within its mental health services yesterday.

In a statement, the executive said it could confirm that “two separate tragic incidents” had occurred Monday.

In what are described as “unconnected incidents”, one patient died at the Acute Mental Health Unit at Kerry General Hospital, while another died at the HSE Acute Mental Health Unit on the campus at Tallaght Hospital.

“Immediate and comprehensive reviews of the circumstances of these two incidents have begun, in accordance with HSE policy,” it said in a statement issued tonight.

Services are offering all available supports to the people involved – to the families concerned, to our other patients and to our staff.

The executive extended its sincere sympathies to the bereaved families.
